Registered, Excepted and Branch of a Charity Form

This form is to be used to allow organisations that are Registered, Excepted, or a verified Branch or affiliate of a charity to provide evidence of their eligibility status (e.g. unique charity registration number or documentation confirming eligibility.

Please note the form must be completed by 30 April 2025.
